For patients with body weight above 40 kg, the loading dose is 150 mg/kg in 200 mL of 5% dextrose (D5W), infused over 60 minutes. The second infusion is 50 mg/kg in 500 mL D5W, infused over 4 hours (12.5 mg/kg/h).
