I. IMPORTANT TESTING INFORMATION FOR APPLICANTS (Applicants are responsible for knowing the testing policies and procedures. Read and fill out this entire form to avoid a delay in your Application .) 1. Please send photocopies of all supporting documents for your testing eligibility to complete your Application . Incomplete material will result in a delay for approval or denial to sit for the exam. (We will not transfer information from old Certification applications .) 2. Applicants that are not approved will receive a refund of the Application fee minus a $40 administrative fee within 2 weeks of receiving notification. 3. applications must be typed or printed neatly in ink. Illegible applications will be returned. 4. Non-Member rate includes one year CWEA membership.
