STATE OF CALIFORNIA HEALTH AND HUMAN SERVICES AGENCY CALIFORNIA DEPARTMENT OF SOCIAL SERVICES COMMUNITY CARE LICENSING DIVISION CRIMINAL BACKGROUND CLEARANCE TRANSFER REQUEST The Department may TRANSFER an individual s CRIMINAL record CLEARANCE from one state-licensed facility to another. Clearances cannot be transferred from a state licensed facility to a county licensed facility, or from county to state. The facility licensee, administrator, or director who is seeking the TRANSFER must verify the individuals identity and include a copy of the person s California driver s license or a valid photo identification issued by another state or the United States government if the person is not a California resident.
