Requirements for Qualified Supports/Services Providers (QSPs) who are DDA Approved and/or Licensed are determined by the Department, and are identified in each waiver’s Application for a 1915(c) Home and Community-Based Waiver. Eligibility requirements for providers have been approved by the Centers for Medicare and Medicaid (CMS).
