1 11/1/17 12:36 PM PACKING INSTRUCTIONSSTEP 1: Print this RETURN AUTHORIZATION form, complete the lower portion and place it in the box with the EQUIPMENT as indicated 2: Please disconnect all devices attached to the DIRECPATH EQUIPMENT being returned. Place the DIRECPATH EQUIPMENT and the power cord in the original EQUIPMENT box. Alternatively, you may utilize: Your own small box (12 x 12 x 4 or less) A free Medium Flat Rate Box available inside your local USPS Post 3: Seal the box and firmly attach the label outside of the box using clear packing OPTIONSOPTION 1: Give the package to your United States Postal Service letter carrier directly, or place in your mailbox for outgoing 2: Drop off the package at any Post Office. To find the Post Office nearest you, call 1-800-ASK-USPS (1-800-275-8777) or go to and then click on Locate a Post Office on that 3: Request a Free Package Pickup from your Postal letter carrier at Your letter carrier will pick up your package when your mail is delivered the next day.
