Important Instructions for China visa Application The China visa Application is below on Pages 2 - 5. Please review these Important notes. The Application form must be typed in ALL CAPITAL LETTERS. Handwritten answers, including corrections are not accepted by the Chinese Consulate. On the form it says If some of the items do not apply, please type N/A or NONE . Certain questions cannot be N/A, if the question has a yes or no answer you must state NO or NONE . N/A is only if the question does not apply to you at all. The following questions may not be left blank or answered with N/A: Please provide your Last, Middle and First Name exactly how they appear in your passport. Do not use nicknames or initials unless displayed as so in your passport.
