The Department of Motor Vehicles is authorized to maintain an information reporting service for driver's license and vehicle registration/title records. Pursuant to NRS all requestors must meet the following criteria: Complete sections A, B, and C, sign, and date Application for Record Information (IR002). Complete, sign, date and have notarized affidavit (IR003) stating your understanding of Nevada Revised Statute Mail completed application, affidavit, and any documentation showing your legal right to the requested information, , work order, court order, bill of sale, title, accident report. Businesses must submit a copy of current Business or State license. If business is based in Nevada must provide State of Nevada business license.
