PROGRESS NOTE #1 Steven Perry 2/15/2011 Mr. Perry is here today for extended visit exam, lab results, pt complains today of URI symptoms and colored sputum. HPI: The patient returns reassessment, feeling quite well. He enjoyed his vacation. He relates no HA, syncope, no exertional chest pain. He has a negative nuclear stress test last summer with Dr Freed. Recent office visit there was normal, niacin prescribed. He is been intolerant of simvastin, Vytorin in the past, although he did fairly well on a small, half dosage. He is willing to try again. His cholesterol is 100 points high, unfavorable ratio.
Feb 15, 2011 · Size: 2-D Measurement – 1.6 mm (length) x 1.8 mm (width) Location: Back – left upper Head and Neck Thyroid Gland Characteristics – normal size and consistency and no palpable nodules Eye: Sclera/Conjunctiva- Bilateral – Normal. Pupil – Bilateral – Normal, Accommodating and Direct reaction to light normal.
