This is partly because care was delivered primarily in hospitals which themselves were developed over the decades to deal with infectious disease and trauma.
Fasting plasma glucose ≥ 7.0 mmol/L.* Fasting is defined as no caloric intake for at least 8 hours7. or 2-hr plasma glucose > 11.1 mmol/L during a 75g Oral Glucose Tolerance Test. The test should be performed as described by W.H.O., using a glucose load containing the equivalent of 75 g anhydrous glucose dissolved in water. or
