Revision of a previous bariatric surgical pro cedure or conversion to another procedure for an adult is considered medically necessary when BOTH of the following are met: • Individual has not obtained adequate weight loss* *NOTE: inadequate weight loss is defined as failure to lose at least 50% of excess body weight
