Previous Vaccination Date: _____ Most Recent vaccination Date: _____ (Must be more than 90 days after previous) CHECKLIST FOR 5-DAY-OR-LESS PROGRAM Owners of pets residing in Hawaii please use: CHECKLIST ONLY FOR RESIDENT DOGS AND CATS DEPARTING FROM HAWAII AND RETURNING FOR THE 5-DAY-OR-LESS PROGRAM All of the listed steps need to be completed in order to qualify for this PROGRAM . If your pet cannot meet all of the following requirements, your pet will undergo quarantine for up to 120 days. Step 1 RABIES VACCINATIONS 1 My pet has been vaccinated for rabies at least twice in its lifetime. 1 These rabies vaccines were administered more than 90 days apart.
1 Required Documentation: (Except for the health certificate, all other documents must be received by the Rabies Quarantine Branch more than 10 days before a dog or cat is scheduled to arrive in Hawaii for direct airport release) 1 Completed Dog & Cat Import Form AQS-278 (must be notarized); 1 Two (2) original rabies vaccination certificates signed by a veterinarian;
