There are documents required in each part that will need to be uploaded in order to complete the application . The application may be pended (saved) if you are unable to complete it before submitting to NCPDP . If you pend the application , you will not lose the data you have input you will be able to retrieve you work from the My Pended Pharmacies Queue. Part 1 contains basic information about your pharmacy such as name, address, contact information, pharmacy FEIN, state board of pharmacy license and DEA License. Part 2 contains credentialing and CMS 455 regulatory information such as ownership details, insurance, sanctions and disciplinary information, compounding and other licensing, certification and accreditations information and documentation.
