Instructions page i (separate from main Packet ) Cub Scout Registration Packet Instructions 1) Please be sure to complete all sections of the following forms: a) Cub Scout Pack 163 Registration Form (3 pages) General Information1 (page 1) Additional Scouting Information (page 1) Photography/Image Release (page 1) Medical Information (page 2) Parent/Guardian Authorization (page 2) Vehicle Information (page 2) Fees Information (page 3) 1 If you are new, we will need you to also complete a BSA National Youth Application form that we will provide to you. b) BSA Cub Scout Pack 163 Parent s Promise (2 pages) Our Pack is run entirely by volunteers and we need you to volunteer to help us run at least one event if not two2. There are three sections to this form. Complete the first if you can be a den leader or assistant den leader. (page 5) Complete the second section if you can help us run one of our major events.
