Download the most current version form the USCIS website. ____ Copy of current Employment Authorization Card (both sides). ____ Copies of approval notices (Form I-797) of prior applications of forms I-821D and I-765. ____ Official photo ID: valid passport (preferred), or government photo ID (cedula, matr cula consular or school ID). ____ Personal check or money order payable to "US DEPARTMENT OF HOMELAND SECURITY" for $ ____ Two passport-size (2 x 2), color photographs APPLICANTS WITH A CONVICTIONS OF ANY KIND (including misdemeanors, and traffic offenses). Please obtain a certificate of "final disposition" from the court where you were convicted or paid a fine.
