Direct Transfer Authorization FormTo set up and activate a link between one of your transactional accounts at Canadian Tire Bank and one of your accounts at another Canadian financial institution (an External Account), please do the following:(1) Complete this form in its entirety and sign the bottom.(2) Send a void cheque or fill out a personalized cheque, drawn on the External Account, made payable to you in the amount you d like deposited into your Canadian Tire Bank account, or send a letter from the financial institution holding the External Account that identifies the External Account (stamped by that institution).(3) Mail to: Canadian Tire Bank, PO Box 3000, Welland, ON L3B Accountholders: Please note that to link a joint External Account to a joint Canadian Tire Bank account by using only one form , both accountholders must sign the form and the cheque and the accountholders must be the same for both accounts.
