Endoscopic Lumbar Sympathectomy for Focal PlantarHyperhidrosis Using the Clamping MethodRafael Reisfeld, MD, FACSA bstract:Surgical treatment for Focal plantar hyperhidrosis is notyet well studied. Bilateral Endoscopic Lumbar Sympathectomy (ELS), using the clamping method, was performed in 63 patientswith Focal plantar hyperhidrosis . Clamps were placed at L3( ) or L4 ( ), with one case at L2. All patients hadimprovement in foot sweating, with achieving totalanhidrosis. Five early cases had to be converted to an opensurgical method. Complications were rare. No sexual problemswere reported by the male patients. Compensatory sweating,already present in those with prior thoracic Sympathectomy (n=56), remained unchanged in and no severe compensa-tory sweating occurred in those who had only ELS. Postoperativepain was minimal. ELS is a viable option in the treatment ofplantar hyperhidrosis , whether after a thoracic Sympathectomy orin primary cases of plantar hyperhidrosis .
