We may be able to assist in yourhome, or provide you with information to help with Program Workswith Families to: Identify needs and create goals Establish a plan to achieve goals Serve as a Guide to community resources Nurture an accessible Support system for caregivers Who is eligible for Services?ADULT PROGRAMThe Caregiver must be providing care to a frail adult,aged 60 or older, or an individual of any age withAlzheimer s disease or a related disorder withneurological and organic brain care receiver must need assistance in at leasttwo activities of daily living (eating, toileting,continence, transferring in/out of bed or chair, bathing,dressing) or require substantial supervision due to acognitive or other mental PROGRAMThe Caregiver must be a relative who is at least 55years care receiver must be a minor child under 18, oran individual (of any age) with a caregiving must be the result of circumstances inwhich the natural parent is no longer able to be theprimary AGENCY ON AGING SERV
