Family Caregiver Support Program offers short-termassistance to help families caring for an older person,60 years or older. We may be able to assist in yourhome, or provide you with information to help with Program Workswith Families to: Identify needs and create goals Establish a plan to achieve goals Serve as a Guide to community resources Nurture an accessible Support system for caregivers Who is eligible for Services?ADULT PROGRAMThe Caregiver must be providing care to a frail adult,aged 60 or older, or an individual of any age withAlzheimer s disease or a related disorder withneurological and organic brain care receiver must need assistance in at leasttwo activities of daily living (eating, toileting,continence, transferring in/out of bed or chair, bathing,dressing) or require substantial supervision due to acognitive or other mental PROGRAMThe Caregiver must be a relative who is at least 55years care receiver must be a minor child under 18, oran individual (of any age) with a caregiving must be the result of circumstances inwhich the natural parent is no longer able to be theprimary AGENCY ON AGING SERVING
