Annex A1 to Version 13 March 2017 MANDATORY PROVIDENT FUND SCHEMES ORDINANCE (CAP. 485) ( the Ordinance ) CLAIM FORM FOR PAYMENT OF ACCRUED BENEFITS ON GROUND OF ATTAINING THE RETIREMENT AGE OF 65 OR EARLY RETIREMENT Please read the following important notes before completing this Form. Filling In This Form (a) This Form is to be completed by any person who wishes to claim for payment of accrued benefits from a registered scheme on the ground of attaining the retirement age of 65 or early retirement only. For a claim made on the ground of early retirement, the scheme member must reach the age of 60 and have permanently ceased all employment and self-employment with no intention of becoming employed or self-employed again.
