The muck skip was twice the excavator’s safe lifting capacity. As a result the excavator toppled into a 10 metre depth hole, landing on the muck skip and a mini excavator in the shaft. Fortunately, none of the three people involved were killed and only the excavator operator was injured. Nobody had received training and no lift plan was in place.
