Department of Health services Division of Medicaid services P-00708B (07/2022) State of Wisconsin Page 1 of 59 IRIS Service Definition Manual The IRIS program is a Medicaid Home and Community-Based services (HCBS) waiver program authorized under 1915(c) of the Social Security Act and approved by the Centers for Medicare and Medicaid services (CMS). The IRIS Service Definition Manual details the long-term care services and supports covered by the IRIS program and qualifications providers must meet to provide such services and/or supports.
The residential service provider and participant must maintain an agreement which specifies the nature and scope of the services provided. Participants may purchase individual services from separate providers. In these cases, residential service providers must also coordinate with those external service providers.
