LICENSE APPLICATION AND INSTRUCTIONS . FOR. FAMILY CHILD care HOMES. This contains INSTRUCTIONS needed to file an APPLICATION for a Family Child care Home LICENSE , and to gain access to and print forms. Family Child care is non-medical care and supervision for children, in the provider's own home, for periods less than 24 hours. A LICENSE may be issued for a Small Family Child care Home or a Large Family Child care Home. A Small Family Child care Home provides care for up to 6 children or for up to 8 children if certain requirements are met. A Large Family Child care Home provides care for up to 12 children, or for up to 14. children if certain requirements are met.
