CCA 11 /2017 1 WELCOME LETTER Dear New Member, Thank you for selecting Community Care Associates, Inc. (CCA-I) as your Health Choice Of Michigan provider. Health Choice is a limited health benefit program and it is not traditional health insurance. We look forward to providing you with the highest quality of service available through the Health Choice program. The first step is to select a Primary Care Physician (PCP) from this DIRECTORY . PCPs are practitioners in the following areas: Family/General Practice, Internal Medicine and Pediatrics (IMPORTANT: See last page for Frequently Asked Questions) Your primary care physician will provide your basic health care services. If you need specialty care, your PCP will coordinate with our case management department for these services. Specialty services require pre-authorization which must come from your PCP. Failure to follow these procedures may be considered self-referral and will not be covered by Community Care Associates,Inc.
