NURSE PRACTITIONER PRACTICE AGREEMENT I. INTRODUCTION PRACTICE AGREEMENT between ( NURSE PRACTITIONER ) and (Collaborating physician) with its place of business at Upstate University Hospital / Upstate University Hospital at Community General incorporated in the State of New York. II. PRACTICE AGREEMENT The PRACTICE AGREEMENT is between the Collaborating physician and the NURSE PRACTITIONER and lists the services to be performed by the NURSE PRACTITIONER . These services are outlined in the Scope of Services, Paragraph III. Both parties agree to the conditions and qualifications imposed on their contractual relationship by the PRACTICE AGREEMENT . III. SCOPE OF PRACTICE The NURSE PRACTITIONER is a registered professional NURSE with advanced preparation in a NURSE PRACTITIONER Program who is certified by the New York State Education Department.
The nurse practitioner is a registered professional nurse with advanced preparation in a Nurse Practitioner Program who is certified by the New York State Education Department. She/he meets the following guidelines as set forth in the
