The purpose of this Policy is to provide clear guidance and instruction to staff with regard to the permanent Transfer of adult and paediatric patients into, out of, and within the Trust sites. Each type of Patient Transfer is now outlined with a specific SOP All patients being transferred must have an up to date assessment of any physical or mental capacity risks All patients being transferred must have a current MEWS/PEWS score and a treatment management plan.
