Its mechanism of action as an anti-anginal agent has not been fully elucidated in humans; however, in vitro studies suggest that perhexiline causes inhibition of myocardial fatty acid catabolism ( by inhibition of carnitine palmitoyltransferase-1: CPT-1) with a concomitant increase in glucose utilisation and consequent oxygen-sparing effect.
