Once your Application has been received, a file will be established and you will be notified regarding any missing documents. Upon approval of your Application , you will be notified by letter and requested to provide your initial license fees. Please note that as of February 17, 2003, the State Board of Physical Therapy Examiners is no longer issuing temporary licenses.
