The delivery of care to meet the needs of a resident is based upon the completion of a comprehensive Assessment and the development of a care plan based upon the Assessment . Documentation of this Assessment process is necessary to assure continuity of care and to identify declines, improvements, or maintenance of a resident's condition. The Assessment process known as the RAI involves the completion of the MDS, the CAAs, and the development of a comprehensive care plan.

When a trigger is entered as the response on a resident’s MDS, additional assessment and review of the care area are required to determine the status of the issue. Thus, the CATs and CAAs form a critical link between the MDS and care planning.
