Short Investigation form Incident Number: E20131122-01 Date: 2/20/15 Engineer: Raymond Cho Utility: Pacific Gas and Electric (PG&E) Address: Geyser Switchyard City: Geyserville County: Sonoma Yes In File N/A Brief description: On November 22, 2013, all three transmission lines relayed at the Geyser Switchyard without interruption to customers. Six bushings on 40 year-old circuit breaker #112 were damaged. By 1009 hours, the transmission lines were placed back in service with switching procedures. My Investigation found that a bushing on the circuit breaker had catastrophically failed and damaged the other five bushings.
