Health and Welfare PlansEffective January 1, 2022 SUMMARY plan DESCRIPTIONThis guide presents basic information about all the health and welfare benefits provided by Harvard University ( Harvard ) under the Harvard University Flexible Benefits plan (the plan ), as of January 2022, and your rights to benefits as a plan participant. The plan is maintained for you and your eligible dependents, as long as you meet the eligibility is the SUMMARY plan DESCRIPTION (SPD) for your benefits under the plan . This SPD and any separate plan documents provided to you by Harvard or any of Harvard s insurance carriers and vendors are intended to comply with the disclosure requirements set forth in regulations issued by the Department of Labor under the Employee Retirement Income Security Act of 1974 (ERISA).
