These systems operate under the assumption that EMPLOYEES will FOLLOW the established rules and PROCEDURES . However, when human factors are not considered in the system, errors and violations made by EMPLOYEES can put the entire management system at risk. There are many factors involved in the reason why EMPLOYEES do not FOLLOW established PROCEDURES .
inattention, forgetting, losing the picture, preoccupation and fixation are entirely natural human reactions to the kind of working environment present in a typical workplace. So far we have mentioned three forms of human error: slips, lapses, and mistakes.
