The purpose of this section is to outline some key issues you should consider when clients are startingWebster-pak. It will help you to discuss patient profile information, frequency of supply, the right pack, delivery andreturn, prescription management, how to use the pack (and more) with your client and/or their carer.

3 This checklist can be kept as objective evidence that a comprehensive consultation has taken can record key details and sign off when section has been designed to answer any possible questions your clients may have aboutWebster-pak. This section can be copied for your clients providing clients with full information you can help overcome any fears or possible unknowns. The booklet provides an overview of the medication managementprocess, a checklist to guide you through the important questions to ask and usageinformation. Also included are: Frequently asked questions for clients, doctors and carers which can be used ifyou feel people need more information Sample patient profiles showing the options for recording patient profileinformation A patient consent form you can use before you get startedIf you wish to share some of this information, you can either photocopy from yourbooklet, print relevant sections from your cd or from the Webstercare website.

5 Once a new client is started, keep a copy of your checklist in a plastic sleeve, alongwith their profile data for further reference.
