1 24/7 grids CDSM by design Email : Telephone no. 01792 463865 The 24/7 grid is a simple but inspiringly visual way to enable a person to design the support around their lives in the most effective way possible.
2 It will help someone and their COS to see where they are utilising their current support and where they could look at other solutions. It is a safe and secure on-line system that gives the opportunity for the encrypted file to be shared within the COS and between professionals to allow them to pool their expertise to get the best possible outcomes for the person they support. The grid when completed, will give an immediate visual representation of a person s typical week. Instantly the COS will be able to see the balance of activities, the way a person is supported to achieve that activity and importantly if any cost attached to it.

The more BLUE you see, reflects how independent a person is becoming] The use of COST FREE support solutions such as Assistive Technology and volunteers is also visually represented so that a Panel can appreciate the effort made to reduce spending The 24/7 Grids hold no sensitive information and are only identified to a person through their client ID number. People who need support and their Families are finding it difficult to know where to start. PERSONALISATION asks people to look more creatively at the way they design their support and the grid tools are a wonderfully colourful and visual way to map out what you want to achieve.
4 The Costing tools work in the background and a summary is visible in the bottom of the screen at al times. It tells you how much you have spent and if you already have a PB, how much you have left! Below [fig1] show examples of a family calculating how much they need to ask services for and [fig2] shows how a family use the grid to help manage an indicative RAS 2. Manage the support type There is a cost to every kind of support we use, even 0 such as family, friends, volunteers and Assistive Technology The tools in the grid will calculate every cost for you if you identify the core hourly rate/sessional rate AND if they are sharing this with anyone else [ 1:2/1:3/ 1 ] By sharing the support during an activity the costs are reduced and could be redirected to other areas in their life The 24/7 grid is easy to use.
5 The 3 simple steps being: 1. 1. Add an activity Double click on the block of time you require to add an activity and complete the pop up box *Add a Name to the activity. * Select a Day and Time span *Choose a Support type /add a new one *SAVE [Fig1] This person below has identified what they want to do each week, what level of support they need and how they plan to sort this support using a mixture of paid and other services.
6 The different colours reflect the cost/support ratios being used. This person who has not approached services before has sourced as much natural support and Assistive Technology as they can first and so is currently approaching Adult social care for only 287 a week . If their needs had been assessed on a points for need system the costs could have been 5x higher, but the grids [when integrated into the community network*] always encourage cost free solutions first and sharing of paid support at every opportunity [Fig2] This grid below shows a RAS allocation of 800 a week. The person & their family have added their weekly grid to community network * found many opportunities to share activities and therefore the support, reducing the weekly spend and extending the use of their budget into other parts of their lives.
7 Ultimately goal being to not require it. In this example, the sleep in Lodger ( Sleep-in presence for a room scheme) saves 268 a week *There is an annual licence cost to register and utilise the matching tools in the community grid network How useful will the 24/7 grids be for care managers? Individually each grid is an inspiring and empowering tool to encourage less reliance on social care funded support. Collectively when the grids are pooled in local community networks*, the grids can match areas of similar activities where someone can pool their personal budget to share the support [where appropriate safeguarding allows] Its only when Individuals and their families/COS can find others to share and pool their budgets will savings be made.
8 Individual social workers who are under a great deal of pressure to reduce spending are forced to keep looking to individual allocations to make savings as they are not yet in a position to offer or suggest any other alternative. In side most individuals and their families there is an innate need to be careful with money that you are responsible for. They too would embrace the opportunity to share an activity rather than have it taken away. The encrypting function in the grid reader is a licensed product and so a grid can be exported to your T DRIVE and emailed between professionals for their specialist input and approval safely until the design and package has been agreed.
9 The grids are unreadable by anyone without a grid reader licence. Finally, the grid can be printed off to add to a file being sent to panel to add a very visual and impacting evidence to any request for funding. How useful would the collative information held in grids be for commissioners? An Individual social worker being able to see all of his/her grids together would be very advantageous. Having the ability to see if there was a trend in the kind of support people were choosing. To see if they themselves were utilising enough cost free solutions in their packages and achieving a good service/community life balance.
10 Seeing where the use of Assistive Technology has been successful and duplicating this else where The commissioners will have yet a further overview of the collective grids and will be able to see if different areas in the county are sourcing support and community solutions in different ways.
