2 Once referred to CCU, MDTA will not accept a Waiver Request and all inquiries should be directed to CCU at read the available Waiver Guidelines below, and submit the Waiver Request Form with evidence and documentation. Take required corrective action a. This means taking steps to ensure that the issue will not occur again. Examples are included in the table below. 2. Pay the video toll(s) a.

4 Video tolls must be paid in full before Civil Penalties will be reduced or waived. b. Pay video tolls due by calling E-ZPass Customer Service at: 1-866-320-9995 or visit a Stop-In Center. 3. Submit the Waiver Request Form and any required documentation a. Gather any required supporting documents as listed in the table below. b. Fill out the form and sign the sworn declaration. This form must be signed by the person named on the Notice of Toll Due or Citation . c. Send the form and any required documentation to MDTA following the instructions on the party forwarding delay or prevention of delivery.
