1 2017-2018 Spring Sports Physicals CDSD Sports Physical forms for the 2017-2018 Spring Sports Season will be the only acceptable forms prior to participation in Sports . All CDSD and PIAA Sports physical forms and evaluations completed by a medical examiner outside of the school offered Physicals must: -Have initial physical must be dated after January 22rd, 2018 OR dated after June 1, 2017 and accompanied by a recertification packet (including Cheerleading). -Be turned in to your school s main office or the Athletic Training Room no later than Wednesday, February 28, 2018. -Include forms, if needed, for Certain Medical Conditions (ex. Asthma) dated for the 2017-2018 school year.
2 - Sports physical forms can be obtained from the Sports Physical homepage on the Central Dauphin School District Website. School Offered Sports Physical Dates You must report with all CDSD and PIAA Sports physical forms filled out completely and signed by the parent/guardian to be considered for a Sports physical evaluation or Sports participation.
