We proudly feature a comprehensive benefits package providing you the opportunity to design your own benefits program according to your needs. 2 Total Rewards / 2022 IMPORTANT PLAN INFORMATION Our benefits plan year is January 1 through December 31. Prior to the beginning of each Plan Year (typically in November) you will have an Open Enrollment opportunity to consider changing your current benefit elections. The benefit elections you make as a new hire (including newly eligible due to status change to a full-time position) or during Open Enrollment will remain in place unless you experience a Qualified Life Event.

5 QUALIFYING LIFE EVENT CHANGES If you request a benefit change mid-year due to a Qualified Life Event the change: Must be requested within 30 days of the event*, Must be consistent with the event. Qualified Life Events Include: Change in Status** Spouse s or Dependent s Open Enrollment Judgment, Decree, or Court Order Enrollment/Ceasing to Be Enrolled in Medicare or Medicaid Benefits are paid via payroll deduction. If you are placed on unpaid leave you must contact People Services and make arrangements to pay any premiums to avoid benefit cancellations. Unpaid benefits are cancelled after 30 days. *Life Event changes become effective the first of the month following completion of the enrollment. **Change in Status includes change in marital status, change in number of dependents (due to birth, adoption, death), change in employment status of the employee, spouse or dependent, dependent satisfying or ceasing to satisfy Plan s eligibility requirements.

6 Benefit enrollments are made online through the Paylocity Enterprise Benefits portal. Link to the Benefits Enrollment Guide available online through 3 Total Rewards / 2022 COMPANY-PROVIDED BENEFITS These benefits are provided to all eligible Associates and are paid by Neighborly . No enrollment is required, and the benefits begin automatically after the required waiting period(s). BASIC LIFE INSURANCE guardian If you die, this plan will pay your beneficiary a benefit equal to 1x your annual base salary, with a minimum benefit of $25,000 and a maximum benefit of $200,000. ACCIDENTAL DEATH & DISMEMBERMENT (AD&D) INSURANCE guardian If you die from an accident, this plan will pay your beneficiary an additional benefit equal to the full amount of your Basic Life Insurance. short -TERM DISABILITY (STD) COVERAGE The STD Plan benefits begin after day 0 of accident or day 7 of illness.

7 The level of income for the disability period, combined with other sources, is 60% of weekly earnings up to a maximum of $1,250 per week. Maternity short -term disability is paid at 100%. Maximum length of benefits is 12 weeks.
