4 The Optional Supplemental Rider coverage is available only to Members who have purchased it under the plan . If you have not previously purchased the Optional Supplemental Rider coverage, you may do so now by contacting our UnitedHealthcare Customer Service at the number listed on the back cover of this from these providers will NOT be covered, if your plan does not include these Additional benefits or you have not signed up for an Optional Supplemental Rider a Primary Care Physician (PCP) You will have to choose one of our network providers that are listed in this directory to be your Primary Care Physician (PCP).

5 The term PCP will be used throughout this directory. Generally, you must get your health care coverage from your PCP. Call the PCP you are interested in selecting to see if they are accepting new patients. If you select a PCP who is not accepting new patients we will automatically assign you to a PCP who is accepting new patients. You are not required to stay with a PCP. You have the option of selecting another PCP that is accepting new patients by calling UnitedHealthcare Customer Service at the number listed on the back cover of this directory.

6 Your PCP plays an important role in your health care. We want you to feel comfortable with the PCP you choose. benefits of partnering with your PCP include: Helping you make smart lifestyle choices to improve and maintain your health Coordinating the care of various specialists you are seeing for specific conditions Managing your prescriptions and identifying potential adverse interactions Helping you understand the complex world of health careYou may want to ask your PCP for assistance in selecting the network specialist. You should always follow-up with your PCP after any specialist visits.

7 You are encouraged to maintain a relationship with your PCP is trained to handle the majority of common health care needs. If you feel the need to see a specialist, your PCP can recommend an appropriate network specialist for your medical condition and answer questions you have regarding the specialist s treatment plan . Your PCP will also provide follow-up health care as network providers listed in this directory have agreed to provide you with your Health Care, Vision Care, Additional dental services, and Hearing services (including Hearing Aid) coverage.

8 You may go to any of our network providers listed in this directory. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network can ask our plan to pay our share of the cost of your covered servicesThere may be times when you get a bill from a network or non-network provider for the full cost of medical care you have received. In many cases, you should not pay the bill but instead you should send this bill to us. We will look at the bill and decide whether the services should be covered.

9 If we decide they should be covered, we will pay the provider directly. Please call UnitedHealthcare Customer Service at the number listed on the back cover of this directory, or see your Evidence of Coverage for more and Urgently Needed CareIn the event of a medical emergency, go get help as quickly as possible. Call 911 for help or go to the nearest emergency room or hospital. Emergency care can always be obtained in or out of the service area from the nearest available provider. We cover emergency services whether you are in or out of the service area.

10 Emergency services are also covered whether or not a contracting medical provider provides them. Our plan covers ambulance services in situations where getting to the emergency room in any other way could endanger your health. You or someone else should call to tell us about your emergency care, usually within 48 you need urgent care when in the service area, you must use plan providers for urgent care. When out of the service area urgent care may be obtained from the first available provider. In addition, when out of the service area you can obtain dialysis treatment from any qualified dialysis provider.
