You can obtain thisnumber from your plan administrator or from your quarterly Create a username and passwordOnce you ve entered your contract number,you will be prompted to create a uniqueusername and password. You will need these to access the website and your account online in the Enter your PINYou will be prompted to enter your PIN. You can find this number on the PIN cardwhich was attached to your enrollment formor mailed to you. Enter this five-digit numberin the space provided.

Go!You will now be able to access yourpersonal account online to check youraccount balance, make fund transfers, or check out the fund lineup available to your plan. Ongoing AccessOnce you have registered online, you will onlyneed your customized username and passwordfor all future visits. If you forget your customizedusername or password, you will be prompted toanswer a security question to regain access. Please note Your PIN card is do not lose or discardit and always keep it in asafe place.

4 If you forget your logininformation, you can alwaysuse the number from yourPIN card and the balance ofa recent statement to gainaccess to your account. If you receive two PIN cards,please use the card with themost recent date.
