1 Making the switch from Anthem to HUMANA should not result in the need to change healthcare providers for most Cincinnati State employees. More than 98% of the Anthem healthcare providers are also within the Human HMO Premier Medical Plan. The small percentage of those not within the Anthem system are typically mental health, chiropractic and other specialty healthcare providers. You can use HUMANA s web-based Physician Finder Plus service to: Look up a person, group, or facility by name, to confirm that a specific PROVIDER is in the HUMANA HMO Premium network using the Find Doctor feature. Get a PROVIDER s phone number, address, or directions. Find the most recently updated list of in-network providers quickly and easily. Search for a PROVIDER by specialty and other criteria, as well as by distance from your home or work. Confirming Your PROVIDER Is Within The HMO Premier Medical Plan 1.
2 It s easy to use Physician Finder Plus. First, go to and click on both the Find a Doctor tab on the main screen and on the insert box that will then open up in the center of the page. 2. In the Search by Coverage & Network box on the right, select Employer Group Plan (Non Medicare), enter your zip code in the space provided, and click Go. 3. Enter your location by either searching for providers near a certain ZIP code or, if you prefer, within a particular county. Don t forget to agree to the Terms of Use on this page, too. 5.
