1 1 ACOs in Your State ACOs DATA PAGE This features a searchable list of organizational information by Accountable Care Organizations (ACOs) as of July 1, 2019. For example, you can find an ACO s service area, start date, and contact information. pageAdditional features are available on top of the website toolbar to help you find or personalize your search results, including ACO data on previous performance years. The About, Filter, and Export functions may be very helpful. ADDITIONAL FEATURES The About function describes the Medicare shared savings Program ( shared savings Program) and includes disclaimers about data. The Filter function automatically populates on the right side of the screen in the dark blue box. Use it to narrow a search by using the conditions provided, or add a new filter condition based on ACO name; ACO address or ACO service area. Adding a new filter condition allows you to select a column to filter. Click on Add a New Filter Condition to view a column name with hyperlink.
2 Click on the hyperlink and select a column to filter by from the dropdown box. Refine your search by using the filter next to the condition. Again, click on the hyperlink to reveal the operation options and select a condition. The Export function lets you download your search results into one of the following file formats: CSV, CSV for Excel, JSON, RDF, RSS, XML Once you have identified the data you wish to download, select the light blue box named Export located above the toolbar. The Export box will appear on the right side of the screen. Select the format for the download. Open or save the format from Once your download is complete, click Done to close the Export feature. ACO PARTICIPANTS DATA PAGE This contains a searchable list of shared savings Program ACO Participants as of July 1, 2019. The ACO Participants are listed with their ACO s organizational information. Use the additional features on the top of the website toolbar to personalize your search for ACO data.
3 Page2 ACO SKILLED NURSING FACILITY (SNF) AFFILIATES DATA PAGE This contains a searchable list of shared savings Program ACO SNF affiliates as of July 1, 2019. ACO SNF affiliates are associated with those ACOs that are approved to use the shared savings Program SNF 3-Day Rule Waiver. The ACO SNF affiliates are listed with their ACO s organizational information. Use the additional features on the top of the website toolbar to personalize your search for ACO data. pag
