Even though not incurred in connection with a disease or injury, Covered Medical Expenses include charges incurred by a female age 35 or over for a routine mammogram, as follows: Routine mammograms for a person age 35 or over are covered once per calendar year.

2 Pap Smear Charges incurred for a Pap smear required in connection with a disease or injury are included as Covered Medical Expenses. Even though not incurred in connection with a disease or injury, Covered Medical Expenses include charges incurred for a routine Pap smear including associated laboratory fees once per calendar year for Preferred Care Providers. Prostate Cancer Screening Even though not incurred in connection with a disease or injury, Covered Medical Expenses include charges incurred by a male age 35 or over in connection with an annual exam for screening for cancer of the prostate, including: a digital rectal exam; and a prostate specific antigen (PSA) test.
