5 AUTHORIZE CHAIRMAN TO SIGN CA 8) Proposed malpractice extension of excess medical professional liability coverage for KMC employed and independent contractor physicians when providing professional medical services at clinical facilities owned and operated by Clinica Sierra Vista (CSV), effective May 18, 2015 (Fiscal Impact: Unknown) - APPROVE AGENDA - board of Supervisors KMC Page 3 Regular Meeting - KMC 5/11/2015 CA 9) Proposed Amendment No. 2 to Agreement 1050-2009 with JB Developers, Inc., an independent contractor, for software services related to the automated employee time and attendance tracking system, increasing the maximum payable by $26,800, from $316,954 to $343,754, to cover the extended term (Fiscal Impact: $26,800 [FY 2015-2016 $7,796]; KMC Enterprise Fund; Budgeted; Discretionary) - APPROVE; AUTHORIZE CHAIRMAN TO SIGN CA 10) Proposed retroactive Amendment No.
