Dental plan Mars is offering a dental plan through Delta Dental for all of its members and their dependents. You must enroll in the Delta Dental plan within 60 days of retirement (or at the end of COBRA coverage) OR wait until the Annual open enrollment period of August 1 through August 31 for an effective date of September 1 each year. There are three different Dental plans to choose from: Low Option Delta Dental Patient Direct (discount plan , available in CO only) Medium Option EPO a feature of Delta Dental PPOSM (limited network, available nationwide) High Option Delta Dental PPOSM (larger network, available nationwide) Monthly rates effective 09/01/2017 08/31/2019 Employee Only $ $ $ Employee and 1 Dependent $ $ $ Employee and 2 or More Dependents $ $ $ EyeMed Vision Care discount plan available to Delta Dental subscribers at no additional cost Vision plan Mars is also offering a vision benefit through Vision Service plan (VSP) for all of its members and their dependents.

2 Coverage is effective on the first of the month following the date that you submit your enrollment information. There are two different vision plans to choose from: Standard Option VSP Standard plan (Colorado Rates) Premium Option VSP Premium plan (Colorado Rates) Estimated Monthly rates effective 01/01/2019 12/31/2019 Employee Only $ $ Employee and 1 Dependent $ $ Employee and 2 or More Dependents $ $ MARS ASSOCIATES Dental, Vision and Long Term Care benefits available to Retirees from Lockheed Martin Corporation (including all business units and predecessor companies) Long Term Care plan MARS is also offering the option of a Long Term Care benefit plan underwritten by a national carrier for all of its members. The carrier will be determined at the time of inquiry, since availability changes. There is flexibility in benefit choices including various elimination periods, benefit durations, and inflation riders.
