While teaching each student to become a confident, competent, defensive, and safe driver, our prime objective is to produce Collision Free drivers. 3 DOL Written and Drive Test *1 hour of skid monster training (see attached sheet) If making payments, you may secure a spot by pre-registering with a $ payment, the balance within 4 weeks of class start date.

4 Pre-registering will guarantee yourself a spot in your desired section. Those students enrolling the first night of class will be given priority by age. Class sizes are limited. 8 ATTENDING_____GRADE_____ PARENTS NAME_____ REQUIREMENTS 1. Student must be 15 years old by the first day of class. Student may not miss more than three scheduled classes or will be dropped from class with no refund. Program must run at least 5 weeks and not exceed 15 weeks after the last day of class.

9 2. Payment: Payment in full or $250 to secure a spot by pre-registering, the balance within the first 4 weeks. $10 a month late fee for unpaid balance. No refunds. 3. Students must complete all learning activities as assigned by the instructor in classroom. Student must reach 80% competency on all testing. Donate for Life will be presented on first day of class. 4. Students must obtain a Driving instruction permit from the Department of Licensing prior to second week of class. Students must successfully complete behind the wheel Driving activities.
