2 School principals are asked to complete and submit the first health and safety assessment via e-mail to Ms. Katrina Vasquez, Head Start Health Coordinator at before August 22, 2014. Subsequent monthly health and safety assessments must be completed by the principal s designee and filed in each Head Start/Early Head Start teacher s data binder. The attached Head Start/Early Head Start health and safety checklists must be used to complete the health and safety assessment of each classroom. Non-compliance items must be addressed immediately. Head Start/Early Head Start programs will be subject to unannounced health and safety monitoring visits from the Department of Health and Human Services and Miami-Dade County .

3 Any outstanding non-compliance item, at the time of a federal/ County health and safety monitoring visit, will require a corrective action plan to be submitted to the Office of Early Childhood Programs.
