1 Checklist for a Consular Report of Birth Abroad (CRBA). American Citizen Services American Embassy Ashgabat this Checklist with you to your appointment, signed and dated. cannot apply for a Report of Birth and passport without a scheduled appointment for each applicant. check appropriate box next to each document that you bring with you. of the documents require you to submit both an original and a photocopy. documents should be place in the same order as the Checklist with the original on top of the copy. A personal appearance by the applicant (the child) is REQUIRED. Non-refundable Application Fee (pay online for eCRBA). The fee for a Consular Report of Birth is $100. The fee for a minor applicant's passport under 16. is $135; for those 16 and over, the fee is $130. The Consular cashier at the Embassy accepts cash (dollars only and credit cards: Visa, MasterCard, American Express). Photos of applicant and parents You will need three identical passport photographs, 2in x 2in (5cm x 5cm) set against a white background for the applicant and two each for the mother and father.

The applicant's parent(s) or legal guardian (preferably the Citizen parent) or a person applying in The enclosed items contain information to help you apply for a Consular Report of Birth Abroad (CRBA), which is considered a basic United States citizenship document, equivalent to a Birth certificate. The CRBA will form the basis of your applications for a passport. They must be 5 cm x 5 cm (square) color passport photos with a white background.

3 We cannot accept computer enhanced or altered photos. - Affidavit of Physical Presence (If only one parent is an American Citizen. See below.). Evidence of Parentage and Nationality - Child's original Turkmenistan Birth certificate (plus English translation). - Parents' passports and 2 photocopies of the biographic data pages - Parents' marriage certificate, if applicable (plus English translation) and 1 photocopy If one of these applies, you will need the following additional documentation. - There is only one American parent, married to a non-American parent: Please submit evidence showing that parent was physically present in the United States (examples: original school transcripts, pay stubs from employer, passport entry and exit stamps etc). The requirement to transmit your US citizenship is a minimum of 5 years physical presence in the US, at least 2 of which were after the age of 14. Generally, no such evidence is required when both parents are citizens.

4 - If one or both parents were previously married: Please submit evidence showing legal termination through death or divorce of all previous marriages. Provide original death or divorce certificates (plus English translation, if needed) and 1 photocopy of each. - If the parents were not legally married to each other at the time of the child's Birth and only one parent is an American citizen: Consult with the Consular Section regarding additional documentation that may be required to transmit citizenship to the child. Both parents and the child are required to appear in person at the Embassy.
