3 programs and Services provided by the City of Los ANGELES Department of Aging are funded by the Older Americans IS THE DOOR-TO-DOOR transportation PROGRAM? A personalized door-to-door transportation service offering a shared ride for the elderly and people with disabilities who are unable to access other modes of transportation . This is a demand response transportation service which is sensitive to the needs and limitations of the elderly and people with disabilities. WHO IS ELIGIBLE? A SENIOR 65 years of age or older, or a person with a disability Individual who is unable to utilize curb-to-curb transit services. Must register with IS SERVICE PROVIDED? Wheelchair accessible vehicles are based at 16 SENIOR multipurpose CENTERS located throughout the City of Los ANGELES .

4 Participants request service, schedule pick- up and drop-off arrangements with the transportation coordinator at the SENIOR multipurpose SENIOR center serving the area where they live. Program participants are personally assisted from the main entrance of their home or building complex into the vehicles. Once the individual arrives at their destination, they are assisted to the building entrace of their drop-off location. HOW MUCH DOES IT COST? A program donation of $.50 per one way trip is suggested. If you schedule more than one destination per trip, each stop is counted as a one way trip. For those agencies that provide trips outside their service area, a donation of $ per one way trip is ARE THE HOURS OF OPERATION?

5 Service is provided Monday through Friday. Hours of operation are generally from 8:30 to 4:30 Contact the SENIOR multipurpose center serving the area where you live for exact times. WHERE DO I CALL FOR ADDITIONAL INFORMATION? Your local SENIOR multipurpose center (MPC) can provide additional information. A list of MPC's along with phone numbers, is printed on the reverse side of this brochure. You may contact the Los ANGELES Department of Aging (LADOA) for information on other transportation services and/or if you are unable to locate your nearest MPC. GOALS OF THE DOOR-TO- DOOR transportation PROGRAM To improve the quality of life for frail elderly and people with disabilities by providing a personalized door-to-door transportation service.

6 To provide a safe and reliable transportation service to those persons who are unable to access other modes of transit. Requiring personalized assistance to access transportation for the Paratransit Program is provided by the City of Los ANGELES , Department of Aging and the Department of transportation . Participants must sign-up and provide certification of need prior to using services.
