2 Please note that only those applicants residing in the following regions may apply at this Consulate General : Limpopo, North West, Gauteng (excluding the Pretoria metropolitan area), Mpumalanga, and Free State. 2. Documentation checklist This checklist and declaration serves to ensure that only those applications that are accompanied by the complete set of supporting documents may be lodged with this Consulate . a) TOURIST visa . Valid South African passport (with a minimum expiry date of 3 (three) months from return date). Passports must have at least 2 (two) free- clean pages. Application form Proof of residence (to be provided with each application). 1 colour passport sized photo. Copy of itinerary Copy of confirmed airline ticket. The port / airport of entry into Italy must be clear.

3 Copy of accommodation voucher or confirmed hotel/apartment bookings in Italy . The bookings/vouchers must cover the full duration of the stay in Italy . On-the-fly bookings by applicants will not be accepted. Copy of medical insurance. The dates on the certificate / document must cover the full trip. Your medical insurance must be equivalent to at least 30,000 (thirty thousand) Euros for emergency hospital and repatriation expenses. If you have paid for your ticket with a credit card, however, medical insurance is included. The certificate must be provided by the underwriter. However, skiing holidays are excluded and you are requested to purchase additional insurance or provide proof of medical aid cover while abroad. Copy of foreign exchange (of parents in case of minor) Foreign Exchange proof to conform with one of the options listed below: copy of bank letter stating available funds on credit card account OR.

4 Copy of bank letter declaring amount of foreign exchange ordered or purchased OR. copy of credit card statement Letter of employment Applicants staying with a private host in Italy require a signed personal invitation letter (as per Ministry of Foreign Affairs format downloadable from our website) and a copy of that host's Italian Identity Card or passport. This letter may be faxed to applicant. If the applicant has been invited by a person residing in Italy , he or she must submit a completed personal invitation form, as well as a copy of the ID or Carta d'Identit of the person inviting. An example is available from our forms section. A copy of the persons Italian passport is also sufficient. Minors travelling unaccompanied require consent to travel from their parents. Minors travelling with only one parent require consent from the other parent signed before a Commissioner of Oaths.

5 This requirement can be waived in case of sole custody, in which case proof must be provided . Copy of both parents ID is also required. All minors require an unabridged birth certificate and a letter from their school stating that the minor is a registered scholar at the school and that the school is aware that the scholar is travelling abroad. An example of an unabridged birth certificate is available from our forms section. A copy of identification booklets/passports of both parents is also required. The administrative fee for processing your application is You can either pay in cash or use a debit card at the Automoney card machine situated in the waiting room. (The amount required per person/per day during their stay in Italy is approximately Euro (thirty Euros)). b) BUSINESS visa .

6 Valid South African passport (with a minimum expiry date of 3 (three) months from return date). Passports must have at least 2 (two) free-clean pages. Application form Proof of residence (to be provided with each application). 1 colour passport sized photo. Return-trip booking or ticket or proof of available means of personal transport Local document demonstrating status as financial-commercial operator ( business licence, Chamber of Commerce certificate, etc.). Travel agenda indicating scheduled business contacts or letter of invitation from an Italian company Copy of medical insurance. The dates on the certificate / document must cover the full trip. Your medical insurance must be equivalent to at least 30,000 (thirty thousand) Euros for emergency hospital and repatriation expenses.

7 If you have paid for your ticket with a credit card, however, medical insurance is included. The certificate must be provided by the underwriter. If you have been invited by an Italian company, it must provide a formal letter of invitation in which the name of the person being invited must appear together with the accommodation and specific purpose for the visit. ie training, buying machinery, testing products etc. There is an example of this letter in our forms section. The Italian company must also provide a document called VISURA CAMERALE. A. sample of this may be downloaded in the forms section on this website. Please note: Originals of all the above may be requested. We kindly request that the applicant provide documentation requested in an orderly manner. The personnel of this Consulate General will not, under any circumstance, do any sorting of documentation presented with application.

8 Each application must have its own documentation. The visa requirements are applicable to EACH application WITHOUT EXCEPTION. Therefore, EACH passport submitted must comply with ALL the relevant requirements. Foreign Passports non South African passports (with exception of EU passports, which do not require a visa to enter Italy ) must have a permanent residence permit affixed in the passport in order to be considered. No exceptions whatsoever will be made. Spouses and Minor children ( < 21 years ) of NATIONALS. Copy of national's passport Copy of marriage or UNABRIDGED birth certificate (proving relationship with National). Foreign nationals visa requests from foreign nationals without permanent residency will not be accepted. Foreign nationals with a permanent residency sticker in their passport must provide the original Permit of Permanent residency issued from the Dept of Home Affairs.

9 This office reserves the right to request further documentation. APPLICABLE TO ALL: I hereby declare that I have in my possession the above visa requirements.
