1 CONTACT INFORMATION 1-866-467-4897 CREATING NEW PATIENT CHARTS QUICK REFERENCE GUIDE ADD BASIC PATIENT INFORMATION 1 Sign in to GoScripts as follows: a Go to b Enter your credentials. 2 On the GoScripts main menu, click PATIENT CHARTS . 3 On the PATIENT CHARTS screen, click Add New PATIENT , unless the PATIENT is already present in your PATIENT CHARTS section. 4 Enter the required information in the fields. Location (Provider s location) First name Last name Gender Date of birth Address Zip code City State Primary phone number Blanket number. Note: This is a freeform field. Enter any extra information relevant to this PATIENT here. ADD INSURANCE INFORMATION 1 In the Insurance Information area, add the primary insurance information as follows: a In the Primary row, in the Insurance Name field, start typing the name of the insurance provider, and then select the appropriate name in the drop-down list. b Enter the insurance number.
2 C Enter the group number if applicable. (OPTONAL) SELECT PATIENT TYPES In the PATIENT Types area, select all the check boxes that apply to the PATIENT . Note: If you select PATIENT types: you can search for patients by PATIENT type you can generate reports by PATIENT type GoScripts will not limit items available to the PATIENT based on PATIENT types selected. Add and Finish: Completes and saves the current PATIENT chart and opens the PATIENT chart . Add and Create Referral: Completes and saves the current PATIENT chart and begins a prescription order for this PATIENT .
