1 Customer Information PackYour guide to completing this form Please complete this Customer Information pack (CIP) in BLOCK CAPITALS and in BLACK INK. Please complete all fields in all sections of this CIP and sign section H located on page 10. Please note that any joint account holders will be contacted separately and requested to complete their own CIP and provide the required certified documentation.
